    Rehabilitación de edificio para uso hotelero

    El objetivo del presente proyecto consiste en, partiendo de un levantamiento básico de un edificio de vivienda-palacete situado en la finca El Campillo del Gordillo y de unos diseños que alcanzan el nivel de proyecto básico llevar a cabo el proyecto de ejecución que corresponde al citado proyecto básico y que serviría para ejecutar el edificio. En este sentido y dado que el edificio en la actualidad presenta diversas patologías el trabajo se ha completado con un estudio patológico a fin de diagnosticar las patologías existentes, plantear las soluciones a las mismas y presupuestarlas en la documentación del proyecto de ejecución que se redacta. El edificio objeto del trabajo en la actualidad es una vivienda-palacete abandonada construida a principios del siglo XX con un diseño de tendencia claramente portuguesa y la propuesta de intervención que se recoge en el proyecto básico es la de transformarlo en Hotel Rural rehabilitando lo que resulta posible rehabilitar en cumplimiento de la normativa urbanística que se recoge en el Plan General Municipal de Cáceres. La intervención solo alcanza al edificio principal no siendo objeto del proyecto ninguno de los edificios accesorios que completan la finca. Es objeto del proyecto también la modificación del proyecto básico facilitado en lo preciso para cumplir con todas las normas que le son de aplicación y que se justifican en la memoria por lo que se aporta el diseño facilitado y como se puede comprobar este se ve modificado por los cumplimientos citados.Grado en Edificación.     Computer aided hearing assessment : towards an automated audiometric tool

    [Resumen]La pérdida de audición consiste en una disminución parcial o total de la capacidad para percibir sonidos que afecta a un amplio rango de población y tiene un impacto negativo en sus actividades diarias. La audiometría tonal liminar es uno de los tests estándard para la evaluación de la capacidad auditiva. Durante la realización de esta evaluación el audiólogo trata paralelamente de identificar pacientes con tiempos de respuesta anormalmente lentos. Esta identificación es relevante pues podría tratarse de un síntoma asociado a alguna patología que debiera ser estudiada. El otro objetivo principal es la evaluación de pacientes con deterioro cognitivo o trastornos graves de comunicación, puesto que no es posible mantener una interacción típica de preguntarespuesta cuando se evalúa su audición. En estos casos, el experto debe centrar su atención en la detección de reacciones gestuales espontáneas al sonido. La subjetividad implicada en la interpretación de ambos objetivos puede afectar a la clasificación, introducir imprecisiones, limitar la reproducibilidad y también producir un alto grado de inter e intra varibialidad del observador. En este sentido, el desarrollo de un método automatizado, objetivo y sistemático para el análisis y clasificación de los tiempos de respuesta y de las reacciones gestuales al sonido es, por tanto, altamente conveniente, permitiendo un diagnóstico homogéneo y relevando a los expertos de esta tediosa tarea. El propósito de esta investigación es el diseño de un sistema automático para la evaluación de las reacciones gestuales y los tiempos de respuesta a través del análisis de secuencias de vídeo grabadas durante el desarrollo de la prueba audiométrica. Por una parte, los tiempos de respuesta se miden detectando el envío de estímulos y la respuesta positiva del paciente (expresada levantando la mano). Por otra, las reacciones gestuales son identificadas analizando los movimientos de la mirada usando dos aproximaciones diferentes. Las diferentes propuestas automatizadas presentadas ahorran tiempo a los expertos, mejoran la precisión y proporcionan resultados objetivos que no se ven afectados por factores subjetivos.[Abstract]Hearing loss is a partial or full decrease in the ability to detect or understand sounds which affects a wide range of population, and has a negative impact on their daily activities. Pure Tone Audiometry is the standard test for the evaluation of the hearing capacity. During the performance of this hearing assessment the audiologist also tries to identify patients with abnormally slow responsiveness by means of their response times to the perceived sounds. This identificacion is relevant since it could be a symptom of any medical condition that should be studied. The other main target is the evaluation of patients with cognitive decline or severe communication disorders, since when evaluating this specific group of patients it is not possible to maintain a normal question-answer interaction. In these cases the expert must focus his attention on the detection of unconscious gestural reactions to the sound. The subjective involved in the interpretation of both aims may affect the classiffication, introduces imprecisions, limits the reproducibility and also a high degree of inter- and also intra- observer variability can be produced. In this manner, the development of a systematic, objective computerized method for the analysis and classiffication of response times and gestural reactions to the sound is thus highly desirable, allowing for homogeneous diagnosis and relieving the experts from this tedious task. The proposal of this research is the design of an automatic system to assess the gestural reactions to the sound and the patient's response times by analyzing video sequences recorded during the performance of the audiometric evaluations. On the one hand, the response times are measured by detecting the auditory stimuli delivering and the patient's hand raising (which corresponds with a positive response). On the other hand, the gestural reactions to the sound are identifed by analyzing the eye movements using two different approximations. The different automated assessments proposed save time for experts, improve the precision and provide unbiased results which are not affected by subjective factors.[Resumo] A perda de audición consiste nunha disminución parcial ou total da capacidade para percibir sons que afecta a un amplo rango da poboación e ten un impacto negativo nas súas actividades diarias.     Toward a better “Person–Environment Fit” through items calibration of the SIS-C

    Producción CientíficaThe Supports Intensity Scale–Children’s Version (SIS-C) is the only available tool to assess extraordinary support needs for children and adolescents with intellectual disability. In past years, several works have proclaimed the need for its ongoing improvement as a measurement instrument. To contribute to this line of research, the goal of this work is to analyze the reliability of the SIS-C and its usefulness to distinguish between different levels of intensity of support needs. To address this, 814 children and adolescents with intellectual disability (M = 11.13 years; SD = 3.41) were assessed using the SIS-C Spanish version. Item response theory analyses were conducted to estimate latent scores and assess measurement quality along the support needs continuum. The SIS-C items showed good overall discrimination and information values, and none showed problems that required their removal or modification. However, all the scales composing the SIS-C showed problems in discerning high levels of intensity of support needs, especially for children and adolescents with severe/profound intellectual disability. This ceiling effect may be an obstacle for both research and practice involving the SIS-C.     Examining the suitability of the list of indicators describing age-related typical support needs

    Producción CientíficaThe list of indicators is a form of training material used for the Supports Intensity Scale—Children’s version (SIS-C). It is aimed at helping interviewers distinguishing between extraordinary and age-related typical support needs in children with intellectual and developmental disabilities (IDD) when implementing the SIS-C, and thus improve supports planning. The aim of this study is to adapt and test the list of indicators’ content validity and rating scale’s functioning in Spain. A total of 222 general education teachers reported their agreement with each indicator description using a 5-point rating scale. A total of 353 of 366 indicators showed evidence of content validity, whereas analyses on the rating scale highlighted the necessity of subsuming one of the scale categories within another. In some cases, especially in the elderly population, the hearing loss may decrease in physical, mental and social well-being capacities. In particular, patients with signs of cognitive impairment typically present specific clinical–pathological conditions, which complicates the analysis and diagnosis of the type and severity of hearing loss by clinical specialists. In these patients, unconscious changes in gaze direction may indicate a certain perception of sound through their auditory system. In this context, this work presents a new system that supports clinical experts in the identification and classification of eye gestures that are associated with reactions to auditory stimuli by patients with different levels of cognitive impairment. The proposed system was validated using the public Video Audiometry Sequence Test (VAST) dataset, providing a global accuracy of 97.12% for the classification of eye gestures and a 100% for gestural reactions to auditory stimuli.     Valorization of soda lignin from wheat straw solid-state fermentation: Production of oleogels.

    This work describes the solid-state fermentation(SSF) of wheat straw with Streptomyces sp. MDG147 and further soda-pulping process to obtain wheat straw soda lignins(WSLs). Subsequently, these WSLs were NCO-functionalized with 1,6-hexamethylene diisocyanate and then dispersed in castor oil to achieve stable oleogels. The WSLs were characterized using standard analytical methods, gel permeation chromatography, Fourier transform infrared spectroscopy,differential scanning calorimetry, and thermogravimetric analysis. Rheological properties of oleogels were determinedby means of small-amplitude oscillatory shear and viscous flow measurements. The enzymatic profile and production oflignin−carbohydrate complexes were recorded along the growth time of Streptomyces, whose life cycle was achieved after 7 days. NCO-functionalized WSL was able to chemically interact with castor oil via urethane bonding, providing oleogels with suitable rheological characteristics. Linear viscoelastic functions and viscosity values of oleogels were higher when wheat straw was submitted to SSF using Streptomyces, turning out in stronger oleogels.Ministerio de Economía, Industria y Competitivida

    Patient with adrenal insufficiency due to a de novo mutation in the NR0B1 gene

    Adrenal insufficiency; Congenital adrenal hypoplasiaInsuficiència suprarenal; Hipoplàsia suprarenal congènitaInsuficiencia suprarrenal; Hipoplasia suprarrenal congénitaObjectives Congenital X-linked adrenal hypoplasia is a rare disease with a known genetic basis characterized by adrenal insufficiency, hypogonadotropic hypogonadism, and a wide variety of clinical manifestations. Case presentation We present the case of a 26-day old male newborn with symptoms consistent with adrenal insufficiency, hyponatremia, and hyperkalemia. Following NaCl and fludrocortisone supplementation, the patient remained clinically stable. 17-OH-progesterone testing excluded congenital adrenal hyperplasia. The rest of hormones were within normal limits, except for adrenocorticotropic hormone (ACTH), which was significantly elevated, and aldosterone, which was below the reference value. Further testing included very long chain fatty acids to exclude adrenoleukodystrophy, the CYP11B2 gene (aldosterone synthase), and an MRI to screen for other morphological abnormalities. All tests yielded normal results. Finally, after cortisol deficiency was detected, expanded genetic testing revealed a mutation in the NR0B1 gene, which led to a diagnosis of congenital adrenal hypoplasia. Conclusions Diagnosis of congenital adrenal hypoplasia is challenging due to the heterogeneity of both clinical manifestations and laboratory abnormalities. As a result, diagnosis requires close monitoring and genetic testing

    Esta publicación recoge 6 estudios de casos sobre las mujeres campesinas, afrodescendientes e indígenas, en diferentes procesos productivos en la agricultura y pesca artesanal en El Salvador, Honduras, Costa Rica, Colombia y Venezuela. En cada uno de los casos estudiados se plantea la necesidad de generar acciones desde los diferentes espacios para garantizar que las mujeres puedan potenciar sus capacidades productivas, así como recibir una retribución justa del esfuerzo productivo que realizan. El objetivo es hacer una contribución al conocimiento para enfrentar la exclusión social, económica y cultural que viven las mujeres productoras rurales desde sus diversas identidades. El análisis de las cadenas de valor orgánicas de la semilla de marañón, el café, la miel y el cacao, así como la de la pesca artesanal y la de salsa de tomate, dejan plasmada la constancia del aporte significativo que están haciendo las mujeres al desarrollo de sus comunidades y al de sus familias

    Oral Anticoagulation and Risk of Symptomatic Hemorrhagic Transformation in Stroke Patients Treated With Mechanical Thrombectomy: Data From the Nordictus Registry

    Introduction: We aimed to evaluate if prior oral anticoagulation (OAC) and its type determines a greater risk of symptomatic hemorrhagic transformation in patients with acute ischemic stroke (AIS) subjected to mechanical thrombectomy. Materials and Methods: Consecutive patients with AIS included in the prospective reperfusion registry NORDICTUS, a network of tertiary stroke centers in Northern Spain, from January 2017 to December 2019 were included. Prior use of oral anticoagulants, baseline variables, and international normalized ratio (INR) on admission were recorded. Symptomatic intracranial hemorrhage (sICH) was the primary outcome measure. Secondary outcome was the relation between INR and sICH, and we evaluated mortality and functional outcome at 3 months by modified Rankin scale. We compared patients with and without previous OAC and also considered the type of oral anticoagulants. Results: About 1.455 AIS patients were included, of whom 274 (19%) were on OAC, 193 (70%) on vitamin K antagonists (VKA), and 81 (30%) on direct oral anticoagulants (DOACs). Anticoagulated patients were older and had more comorbidities. Eighty-one (5.6%) developed sICH, which was more frequent in the VKA group, but not in DOAC group. OAC with VKA emerged as a predictor of sICH in a multivariate regression model (OR, 1.89 [95% CI, 1.01–3.51], p = 0.04) and was not related to INR level on admission. Prior VKA use was not associated with worse outcome in the multivariate regression model nor with mortality at 3 months. Conclusions: OAC with VKA, but not with DOACs, was an independent predictor of sICH after mechanical thrombectomy. This excess risk was associated neither with INR value by the time thrombectomy was performed, nor with a worse functional outcome or mortality at 3 months

    Changes in humoral immune response after SARS-CoV-2 infection in liver transplant recipients compared to immunocompetent patients

    The protective capacity and duration of humoral immunity after SARS-CoV-2 infection are not yet understood in solid organ transplant recipients. A prospective multicenter study was performed to evaluate the persistence of anti-nucleocapsid IgG antibodies in liver transplant recipients 6 months after coronavirus disease 2019 (COVID-19) resolution. A total of 71 liver transplant recipients were matched with 71 immunocompetent controls by a propensity score including variables with a well-known prognostic impact in COVID-19. Paired case-control serological data were also available in 62 liver transplant patients and 62 controls at month 3 after COVID-19. Liver transplant recipients showed a lower incidence of anti-nucleocapsid IgG antibodies at 3 months (77.4% vs. 100%, p <.001) and at 6 months (63.4% vs. 90.1%, p <.001). Lower levels of antibodies were also observed in liver transplant patients at 3 (p =.001) and 6 months (p <.001) after COVID-19. In transplant patients, female gender (OR = 13.49, 95% CI: 2.17-83.8), a longer interval since transplantation (OR = 1.19, 95% CI: 1.03-1.36), and therapy with renin-angiotensin-aldosterone system inhibitors (OR = 7.11, 95% CI: 1.47-34.50) were independently associated with persistence of antibodies beyond 6 months after COVID-19. Therefore, as compared with immunocompetent patients, liver transplant recipients show a lower prevalence of anti-SARS-CoV-2 antibodies and more pronounced antibody levels decline
